Professional Services

Most people have thoughts and feelings about themselves that are confusing and unresolved. Consequently, behind our everyday social masks may lay anxiety, depression, loneliness, meaninglessness, or a general sense of emptiness-- professionally, emotionally, and spiritually.

 

Our fast-paced society rarely reaches into these parts or ourselves.  Psychotherapy has proven to be an effective way to help identify our unconscious needs and desires and learn to address them.  Whether these might result from childhood difficulties or more recent life experience, we can learn to change old destructive patterns and discover more creative, rewarding ones.

Policies

Fees

Fees are set on a sliding scale basis using gross income and the size of the family unit as a barometer. The fee is also based on the assumption of weekly sessions. Should income change or your session frequency change, we will adjust the fee accordingly. Fees are reevaluated on an annual basis.  We want to offer affordable fees, but without reasonable reimbursement we cannot stay open.  Our fees range from $35 per hour to $120 per hour for individuals and $40 to $125 per hour for conjoint/couple sessions.

Confidentiality

All information disclosed within sessions is confidential and may not be revealed to anyone without your written permission except where disclosure is required by law. Disclosure may be required under the following circumstances: Where there is reasonable suspicion of child or elder abuse; where there is reasonable suspicion that the client presents a danger of violence to others or where the client is likely to harm him or herself unless protective measures are taken. Disclosure may also be required pursuant to a legal proceeding.

Cancellation

Since the scheduling of an appointment involves the reservation of time specifically for you, a minimum of 24 hours notice is required for rescheduling or cancellation of an appointment. The full assigned fee will be charged for sessions missed without such notification.

Payment

Patients are expected to pay for services at the time they are rendered. Please notify your therapist if a problem arises during the course of therapy regarding your ability to make timely payments. Checks should be made out to Coldwater Counseling Center.

Insurance

Patients who carry insurance should remember that professional services are rendered and charged to you and not the insurance company. We will provide you with an Attending Psychotherapist's Statement as a receipt, which you can submit to your insurance company for reimbursement.

To keep fees as low as possible and since we have no regular office staff to handle the paper work we do not belong to any Insurance Provider Panels or Medicare or MediCal.  Patients must contact these entities directly to inquire about receiving benefits.

Emergency Procedure

If you need to contact your therapist between sessions, you can leave a message on our Voice Mail Box and your call will be returned. Please discuss with your therapist what to do should you feel an emergency situation might arise.

Good Faith Estimate (GFE)

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

  • Make sure to save a copy or picture of your Good Faith Estimate.
